Anthem Blue Cross and Blue Shield announced today that it received approval from the Centers for Medicare and Medicaid Services to begin offering Medicare Advantage HMO and PPO products in twelve Indiana counties beginning August 1, 2005, with member coverage becoming effective September 1, 2005.
